On October 28, 2020, the Allegheny County District Attorney’s Office approved charges and a warrant was issued for the arrest of Zachery W. Foltz of South Park. Foltz has since been taken into custody and arraigned on charges that include three homicide related charges: Homicide by Vehicle While Driving Under the Influence, Homicide by Vehicle, and Involuntary Manslaughter. Additional charges include Driving Under the Influence (alcohol and drugs) and drug possession. These charges are in connection with a motor vehicle crash earlier this year that caused the death of Jefferson Hills Police Officer Dale T. Provins, Jr.
According to the affidavit of probable cause, on June 3, 2020 at approximately 8:15 AM
, on-duty Jefferson Hills Police Officer Dale T. Provins, Jr. was involved in a vehicle crash with a vehicleoperated by Foltz in the 200 block of Old Clairton Road in Jefferson Hills. Investigation revealed that Foltz was traveling at a high rate of speed southbound on Old Clairton Road, crossed over into the northbound lane and struck Officer Provins’ northbound police vehicle head on. Both drivers were transported by medical helicopters to a trauma center in Pittsburgh. Officer Provins succumbed to his injuries and died 10 days later.The Allegheny County Office of Medical Examiner ruled the cause of death was as a result of blunt force injuries of trunk and extremities due to motor vehicle accident.
In addition to being a 15 year veteran of the Jefferson Hills Police Department, Officer Provins, a U.S. Marine Corps veteran, was buried with honors on June 20 at Round Hill Cemetery, Elizabeth, PA.
